What is and where can I obtain a statutory declaration form?

A statutory declaration (also called a stat dec) is a written statement that allows you to declare something to be true. Such forms include:
Queensland Statutory Declaration
Commonwealth Statutory Declaration (PDF 89k)
Digital and downloadable print copies of the Queensland Statutory Declaration are available via the Queensland Government publications portal.
A statutory declaration must be signed by an appropriately qualified person as described on the declaration.
Note: it is a chargeable offence to intentionally make a false statement in a declaration.
