Who can issue my medical certificate and what information should it include?

To take some pressure off you during the coronavirus pandemic, Griffith has made some adjustments to the requirement for a medical certificate.
To assist students who need to apply for an assessment application on medical grounds, due to testing positive to COVID, but are unable to obtain a medical certificate, the following is deemed to be acceptable:
documentation received from a relevant state government health authority (e.g Qld Health) when you report a positive RAT or a positive PCR test result.
For those students who are not impacted by COVID-19, the following standard requirements apply:
The medical certificate is to be provided by a registered medical or dental practitioner during a medical consultation (this includes Telehealth). Medical certificates issued by pharmacists or Chinese herbal doctors will not be accepted. It should include the following:
• the date the assessment is due or exam scheduled
• the date you were examined by the practitioner
• statement that in their professional opinion you were "not fit for duty", suffering from "a medical condition" or were "unfit to sit an assessment item on the relevant day"
• your medical practitioner’s contact details, including their provider number
• your medical practitioner's signature.
Please ensure you arrange an appointment witha doctor prior to or on the day your assessment is due or the exam is scheduled. If you are unable to get an appointment with your doctor prior to or on the day your assessment is due, you should use a house or after-hours doctor service.
Important: Disciplinary action will be taken if you are found to have submitted a falsified or fraudulent medical certificate to the University, this includes you altering any detail of the certificate. If there is an error on the medical certificate, ask the doctor to reissue the certificate.
